How to Fill University of Phoenix Application Form

  • Fill out the online application on the University of Phoenix website.
  • Be sure to select “Admissions” from the drop-down menu, then select “Undergraduate Students.”
  • Provide your personal information, including name and address details as well as contact numbers such as phone number, email address etc., which will be used for communication purposes between you and University of Phoenix admissions department if you are selected for admission into any college within this institution (which may include Graduate School).
  • You also need to provide information about previous academic history along with GPA/GPA range so that they can assess whether or not you meet their requirements for admission into specific programs/colleges within their university campuses across United States.* If any other forms are required such as financial aid forms then simply click on appropriate links within this page where applicable; otherwise continue filling out remaining sections until completion.* Once completed print out copies for yourself before submitting completed form via mail OR faxing them back at 1-800-921-9111

Step 1: Enter personal information

The first step to applying to the University of Phoenix is entering your personal information. This includes:

  • First name
  • Last name
  • Email address (you must provide one)

You will also need to enter your birthdate and gender, as well as marital status and address information. You’ll need to provide the following:

  • City, state, zip code

Step 2: Answer questions about your high school courses and grades

The next step of filling out the University of Phoenix application form is to answer questions about your high school courses and grades.

  • To do this, you’ll need to know how to calculate your GPA (grade point average) as well as how many credits were required for each course in order to calculate the total number of credits you have earned.

Step 3: Provide information about your military experience, if applicable

If you are a military veteran, provide your DD-214 form.

If you are a military spouse, provide your spouse’s DD-214 form.

If you are a military dependent and your parent(s) or guardian(s) served on active duty in the U.S. Armed Forces and were killed or died as a result of combat-related injuries during their service, provide their death certificate with photocopies of all pages that show the cause of death (front and back). If there is no record of their deaths or if they do not qualify for this benefit based solely on their time in service but they died while on inactive duty status due to illness or injury unrelated to combat injuries (such as heart attack), we may be able to help by reviewing documentation that supports why we should consider awarding benefits based on “unusual circumstances” instead

Step 4: Identify what you have already accomplished

This is where you can brag about all of your achievements. If you were a member of the student council, list it. If you have won an award or honor, list that too! If there’s anything else that makes you stand out as an individual and can be added to this section, include it here as well.

  • Volunteer work
  • Awards/honors/scholarships received during high school (sentence or two)
  • Leadership positions held in clubs and organizations (sentence or two)

Step 5: Agree to the terms and conditions.

You must agree to the University of Phoenix’s terms and conditions in order to apply. The University of Phoenix is committed to providing a safe and secure campus environment for all students, faculty, staff, and visitors. Applicants are required to read and accept the following terms and conditions in order to comply with this commitment:

  • Contact information (phone number/email address) provided in your application must be accurate, social security number(s) or other unique identifiers used on any financial aid forms provided by third parties such as lenders or government agencies; if incorrect information is provided by an applicant then they may be subject to disciplinary action up until graduation or completion of their program requirements whichever occurs first!

Step 6: Submit your application and pay the application fee.

In the final step, you’ll need to submit your application and pay the application fee. Once you have completed all six steps, you can click on “Submit Application” at the top of the page to submit your application. Your information will be confirmed on a confirmation page and you’ll receive an email confirmation as well..

Now that all of your information has been entered into their system, it’s important to remember that if anything changes (such as addresses), then it needs to be updated in order for them not lose track of us when we start classes!

Filling out an application form can be difficult but not if you take it step by step.

The University of Phoenix Application Form Requirements are as follows:

  • Application Fee for University of Phoenix: $50 USD
  • What is the Application Fee for University of Phoenix? The application fee is non-refundable and can only be paid by credit card or electronic check. The amount charged will be equal to one-half (1/2) of the tuition cost per credit hour, which is currently $1,050 USD. This means that if you’re applying for a degree program that takes 60 credits total (30 per semester), then you’ll pay $1750 USD in application fees alone! That’s quite expensive but nothing compared to what it costs to go through college nowadays…

University of Phoenix is a great place to get your degree and start your career, but it is also important that you know what you are getting into before filling out an application form. Making sure all the information on the form matches what they have on file from previous years or from other schools where you may have attended is the first step in this process.. Once this has been confirmed then it’s time to fill out each section individually so that everything looks professional when submitted!
